I use the MCA104 here, which is quite similar to the MCA104M.

Beware of the sticker shock though, as prices have gone up.

Do you use the supplied power supply or do you use something else for
the +12V DC?

The one I have that I purchased on eBay did not come with a power supply. I
currently use a wall-wart.
